Model Community Benefit Programs

A significant component of the ASACB Pilot Program is a comprehensive review of the participating hospital sites’ community benefit programs. One of the products of that review is an Enhancement and Monitoring Template. The Template provides a brief description of program activities, and then outlines the enhancement strategies, measures, and specific steps taken by pilot site hospitals to better align existing programs with the five ASACB Core Principles.

In the comprehensive review process, pilot sites collect baseline information on existing programs to assess relative alignment with ASACB Core Principles, and then develop enhancement strategies to increase alignment. They then use the Template to identify measures that provide evidence of the successful implementation of the enhancement strategy, and action steps that will achieve the desired measurable impact.

In short, the Template serves as a concise, detailed strategic action plan to increase the effectiveness of community benefit programs.

100% School Campaign: Orange County’s Children’s Health Insurance Initiative – the 100% School Campaign is part of a larger countywide initiative whose goal is to insure every child in Orange County, California through the enrollment of all eligible children in governmental programs. For those children who are not eligible for government programs due to residency status, the program partners are raising funds for insurance premium support. 1 |  2 .

Community Grants Program – The Community Grants Program awards funds to community-based organizations that assist underserved communities. Grant criteria focus on Community Benefit priority areas and support alignment with the five ASACB Core Principles.  1 |  2.

Community Health Education – Community based health education supports a variety of hospital programs, including: senior health, breast health, cardiac health, childbirth classes, hospice education, health fairs, support groups, and screenings.  1 |  2.
Presbyterian Intercommunity Hospital, Whittier, CA

Community Wellness Program – The Community Wellness Program provides community-based screenings, health education classes and in-home health education visits that focus on diabetes and obesity. The program utilizes a van that carries health education and screening supplies to a variety of sites in low-income communities; monthly, the program has six regularly scheduled stops. 1 |  2 .
Mercy and Memorial Hospitals, Bakersfield, CA

Glide Health Clinic – The Glide Health Clinic is located at the Glide Methodist Church in the Tenderloin District of San Francisco. This primary care community clinic offers medical, mental health, and podiatry services to adults. The Glide Health Service offers HIV/AIDS education, testing and counseling in the Tenderloin and other poor, high-risk communities. 1 |  2 .

Healthy Silicon Valley Hospital Taskforce – A community-based hospital partnership to address health-related issues associated with obesity in the Silicon Valley. The program goals are to change food and physical activity environments through policy and practice changes within the health care system and community environments through internal, taskforce level, and community partnerships. 1 |  2 .
Lucile Packard Children's Hospital, Palo Alto, CA

Healthy Promotions Dental Program – Healthy Promotions provides dental examination, deep teeth cleaning, restorative treatment and oral health education to low-income, poverty level, and homeless adults. The program screens applicants for care, schedules treatment with dental hygienists and dentists, and offers transportation, translation and funding to cover dental treatment costs. 1 |  2 .
Mercy and Memorial Hospitals, Bakersfield, CA

Homecoming Services Program – a collaborative effort between Saint Francis and community-based organizations to assist isolated seniors upon discharge from the hospital. A social worker and case manager assist seniors to obtain needed services during the initial phase of their in-home recovery, helping seniors remain independent and avoid re-hospitalization and institutionalization. 1 |  2 .

McMillan Stabilization Project – A medically supervised sobering center that hosts 40-50 patients at a time and is designed to divert intoxicated persons from the ED to a 24-hour central facility providing medical screening, case management services to engage clients in recovery programs and services, and linkages to a comprehensive continuum of care. The medical staff from the Department of Health Services provides oversight. 1 |  2 .

Parent Information Referral Center (PIRC) Nurse Advice Line and Teen Line – PIRC is a free telephone service staffed by pediatric nurses that offers parents, child care providers and physicians answers to pediatric health care questions and offers information on available community resources for children. The Teen Line provides resources and confidential answers to health questions for high school aged youth. 1 |  2 .

School-Based Childhood Obesity Program – In 2007, all elementary schools in Texas will be mandated to provide wellness programs to their students. Coleman Elementary is working in partnership with Harris Methodist Walls to offer a coordinated response that includes effective prevention and treatment of childhood obesity. They are providing a school-based intervention approach that includes: physical fitness activities, health education, healthy cafeteria food offerings, and other extracurricular activities that seeks to promote the general wellness of all students. 1 |  2 .

Senior Falls Prevention Program – A 16-week program of education and exercise that targets seniors. The program is held at senior centers in eight cities and has an in-hospital component at Presbyterian Intercommunity Hospital. Patients who are admitted due to a fall or sustain a fall while hospitalized are referred to the program, however, all participants in the Falls Prevention program are not from referrals, but rather a result of advertising through the Senior Services Department or physician referral. 1 |  2 .

Youth Exercise Program – a community-based youth exercise program located within a low-income community. The program is implemented through the Richman Park Community Center and provides opportunities for children, ages 6-18, to engage in formal sports teams, individual exercise, group exercise classes and basic health/nutrition classes. The Valencia Health Committee is led by community stakeholders. This Committee acts as an advisory board to the youth exercise program. 1 |  2 .

Youth Wellness Program – The Youth Wellness Program is an eight-week program that serves youth, ages 12-18, at a local community Family Focus Center. The program provides the kids with lifestyle education and hands-on learning about healthy eating, physical fitness and community advocacy. 1 |  2.
