Call for Breakout Session Proposals
We invite you to submit a proposed breakout session between July 12 and September 1, 2006. Please read the topic track, session format and selection process information below before submitting a proposal.
The Association for Community Health Improvement (ACHI) is seeking session leaders who have successful experiences, tools and approaches to share, and who are willing to both present and engage in dialogue with the audience. Interesting, engaging and enjoyable presentations are the most successful at conveying information participants will use.
When you are ready to submit a proposal:

- Select "create new account"
- Follow the on-screen instructions to enter a session title, learning objectives, brief description and presenter information.
- Once you establish an account, you will be able to save partially completed work and finish or edit it any time through September 1.

Conference Topic Tracks and Cross-cutting Themes
ACHI is soliciting session proposals in four topic tracks. All proposals must directly fit one of the tracks, and may indirectly fit one or more additional tracks.
- Maximizing Access to Care: Discuss and debate local and state approaches to making health care more accessible. This track will include not only coverage and care for the uninsured, but also cultural, linguistic and transportation access, health literacy, and the role of policy.
- Innovating for Community-based Chronic Disease Prevention: Share and explore successful community-based and health system approaches to creating the conditions for health that will prevent and reverse the course of chronic illness. Examine topics of obesity, diabetes, heart disease, asthma, with particular attention to resolving disparities.
- Achieving Community Benefit Excellence: Learn from hospitals and health systems that are leading improvements in community benefit practice. Come see how hospitals are structuring community benefit departments, defining job descriptions, adding community benefit to their strategic plans, linking it to other hospital departments, involving governance and connecting with community.
- Leading Community Health Assessments with Impact: Join this track for exposure to the pros and cons of different assessment models, successful data collection and presentation strategies, and priority setting and action planning. Discuss the importance of identifying the purpose of and audience for your assessment, the financial and human resources to make it happen, and using it to stimulate discussion and change.
As you prepare your proposal, please keep in mind that we particularly value entries that also address one or both of the conference's cross-cutting themes:
- Advocating for Community Health, and
- Building Partnerships among Health Care Leaders.
Session Formats
All breakout sessions are 1 hour, 20 minutes in length. We invite proposals for two types of sessions:
- Interactive Breakouts: Sessions with a little presentation and a lot of facilitated dialogue, group exercises and/or exploration of new ideas.
- Presentation Breakouts: Sessions with formal presentation followed by audience discussion.

Note: Interactive Breakout proposals will be reviewed in part on the strength and specificity of their interactive design, including a role for facilitation, discussion questions and/or group exercises.
Introductory/Advanced Levels
Help us ensure a mix of educational opportunities for both early-career and advanced professionals. Mark your proposal with the most appropriate experience level of the intended audience.
- Introductory/Intermediate: Intended for people with zero to five years of experience in the subject or field. Introduces concepts, definitions and how-to steps.
- Intermediate/Advanced: Intended for people with more than five years of practical experience in the subject or field. Emphasizes advanced concepts and techniques based on practice.
Submission and Selection Process
We invite you to submit a proposed breakout session between July 12 and September 1, 2006. Please read the topic track, session format and selection process information before submitting a proposal.
- All submissions will be made online through this page.
- A volunteer panel and staff will review all proposals. All people submitting proposals will be notified whether they are invited to present on or about Friday, October 13. We anticipate receiving more high-quality proposals than we will be able to accept.
- Membership in the Association for Community Health Improvement is not required to submit.
- Individuals can submit more than one proposal.
- Authors of accepted proposals will be asked to sign a Presenter Agreement to confirm their participation.
- The conference registration fee for breakout session presenters is a discounted $175 (approx. 50% less than the member rate and 66% less than the non-member rate).
- Presenters are responsible for their own travel and accomodations expenses.
