ACHI members: You will need your membership number when registering, in order to receive the member discount. Locate your member number on any recent edition of Community Health News, by logging into your MyACHI account at www.communityhlth.org or by writing to us at communityhlth@aha.org.
Conference faculty, exhibitors and sponsors: Please register only after receiving your specific registration form from ACHI staff. Do not use the form posted above.
