Concurrent Breakout Sessions
Call for Breakout Session Proposals
The Association for Community Health Improvement (ACHI) is committed to producing professional education conferences of the highest caliber. We invite you to join us in delivering a learning experience that stimulates real change and real improvements in how community health programs are planned, run and evaluated.
We are looking for excellent stories of community health improvement initiatives, and also for demonstrated outcomes and specific guidance for how conference attendees can take home what you have achieved and replicate it.
To this end, ACHI’s Conference Advisory Committee and staff will evaluate proposals according to the following criteria:
- Relevance of proposed session to the description of one or more conference topic tracks.
- Clarity and relevance of learning objectives.
- Presence of specific, measurable outcomes from the described program and/or a description of evaluation metrics and methodologies.
- Presence of a description of how - with practical strategies, steps, worksheets or tools - participants can replicate or adapt your success.
In addition, ACHI will look favorably on session proposals that describe beneficial changes to local health care or public health systems, or that address fundamental causes of poor health. We seek to offer sessions on both direct services and systems change initiatives.

Conference Topic Tracks
ACHI is soliciting session proposals in five topic tracks. All proposals must directly fit one of the tracks, and may indirectly fit one or more additional tracks.
- Minimizing Chronic Disease: Strategies for Social Determinants:
Chronic disease prevention and management strategies that address multiple determinants of health, including environmental, social, economic, behavioral and biological factors.
- Maximizing Coverage: Proven Programs and Innovative Proposals:
Successful practices and emerging approaches for expanding health care coverage at the local and state levels, such as enrollment and outreach strategies and new funding schemes, particularly in light of the current national debate.
- Harnessing Data: Health Assessments to Health Outcomes:
Community health assessment strategies and methods, including planning an assessment, collecting data, setting priorities and promoting action. The use and interpretation of health outcomes metrics and measurement tools focused on communities and populations.
- Delivering Community Benefit: Management Tools and Community Strategies:
(co-sponsored by Catholic Health Association and VHA, Inc.)
Leading practices in community benefit program planning and management, including the structure and roles of community benefit departments, defining job descriptions, incorporating community benefit into strategic plans, linking it to other hospital departments, involving governance and connecting with community.
- Breaking Silos: Public Health and Health System Partnerships:
Tangible examples of and critical success factors for building effective public health partnerships with health care and community organizations, including shared resource allocation and decision making, and the use of public health tools and methods across sectors.
Session Formats
Sessions are 1 hour, 20 minutes in length. We invite leaders for two types of concurrent sessions:
- Interactive Breakouts: Sessions with a little presentation and a lot of facilitated dialogue, group exercises and/or exploration of new ideas.
- Presentation Breakouts: Sessions with formal presentation followed by audience discussion.

Submission and Selection Process
Proposals can only be submitted online by Friday, August 3, 2007. No proposals will be accepted by mail, fax, or e-mail. Proposals that are not complete or do not follow the online submission guidelines will not be considered for selection.
The Conference Advisory Committee and staff will review all proposals. We anticipate receiving more high-quality proposals than we will be able to accept.
Note: Interactive Breakout proposals will be assessed in part on the strength and specificity of their interactive design, including a role for facilitation, discussion questions and/or group exercises.
Membership in the Association for Community Health Improvement is not required to submit.
Authors of accepted proposals will be asked to sign a Presenter Agreement to confirm participation.
The conference registration fee for one presenter per accepted proposal will be a discounted $175 (approx. 50% off normal registration). All presenters are responsible for their own travel and accommodations expenses.
